Community Programs

ESLC’s Community Programs are designed to enhance the lives of older adults through three key components: The Neighborhood Programs, offering social, health and wellness activities to keep participants active and connected; Service Coordination, providing personalized support to access essential resources and services to assist individuals in their desire to live at home; and Supportive Housing Services, delivering tailored assistance to those residing in ESLC’s and partner affordable housing; Community Dementia Services, offering the Guiding an Improved Dementia Experience (GUIDE) model, that provides person-centered dementia care to people living with dementia and their family caregivers.

Together, these programs foster independence, well-being, and a strong sense of community for the seniors we serve. To learn more about each of these programs, please see below.

Neighborhood Programs

The Programs are offered at nine campuses, open to all seniors in the surrounding neighborhood. Each promote health and wellness through a tailored balance program, nutrition education, informative presentations and other activities.

Service Coordination

Service Coordination aids in the development and implementation of a person-centered plan that supports the individual's independence, integration into the community and continued health and well-being allowing them to stay home longer.

Supportive Housing Services

Supportive housing combines affordable housing with on-site services to help individuals facing challenges like homelessness, disabilities, or chronic health conditions maintain stability and independence. Supportive Housing Services include case management, health care access, and skill-building, fostering a safe and sustainable environment.
