Community Programs

ESLC’s Community Programs consist of three different aspects to serve seniors, The Neighborhood Programs, Service Coordination and Empire State Supportive Housing Initiative (ESSHI). The Neighborhood Programs, introduced in 2012, were created to enhance the lives of people as they grow older. The program is offered at eight campuses and is open to all seniors in the surrounding neighborhood. Each program promotes health and wellness through a tailored balance program, nutrition education, strength training, informative presentations and other activities. The Service Coordination Program connects clients with in-home support and services through Medicaid’s Nursing Home Transition and Diversion Waiver (NHTD), the Traumatic Brain Injury Medicaid Waiver (TBI) and Empire State Supportive Housing Initiative (ESSHI). This service aids with the development and implementation of a person-centered plan that supports the individual’s independence, integration into the community and continued health and welfare to allow them to stay home longer. Empire State Supportive Housing Initiative (ESSHI) is designed to provide stable housing and rental assistance for seniors who are Medicaid-eligible and require some support services to live safely and independently. ESLC is proud to provide service coordination, health and wellness programs and transportation to seniors who reside at The Gardens at Penfield Square and 270 on East. ESSHI is designed to give stable housing assistance to seniors who are Medicaid-eligible and need support to be independent. ESLC provides service coordination, health and wellness programs and transportation to residents at The Gardens at Penfield Square and 270 on East.
