ESLC’s Service Coordination is provided through Medicaid’s Nursing Home Transition and Diversion (NHTD) Waiver, and Traumatic Brain Injury (TBI) Waiver programs. These programs were developed based on the philosophy that individuals with disabilities, or seniors who require extra assistance, may successfully reside in their home with support from accessible Home and Community Services.
The individual is the primary decision maker, ESLC Service Coordinators work in cooperation with care providers to develop a plan of service that promotes personal independence, greater community inclusion and participation in meaningful activities and services according to the wishes and needs of each participant. Program participants must have Medicaid, be over the age of 18, and require a skilled nursing service.

ESLC's GUIDE model is a new program provided through Medicare (CMS) that supports people living with dementia as well as their families. Those enrolled in this program will have access to a dedicated care navigation team, caregiver support, respite care, & a skilled medical team. Additional services include a 24/7 support line & community education programs.

Supportive Housing Services include case management, health care access, and skill-building, fostering a safe and sustainable environment.
