Supportive Housing Services

Episcopal SeniorLife Communities is proud to offer supportive housing services through the Empire State Supportive Housing Initiative (ESSHI). This initiative is designed to provide stable housing and rental assistance for older adults who are Medicaid-eligible and require some support services to live safely and independently. ESLC provides service coordination, health and wellness programs and transportation to seniors who reside at The Gardens at Penfield Square, 270 on East and St. Leo’s Senior Apartments.

Eligibility

To be eligible for the supportive housing services below, individuals must be unhoused and living in a shelter or on the Continuum of Care prioritization list. Be 55 years of age or older, have active Medicaid and have a chronic condition or physical disability that is documented by a medical professional.

The Gardens at Penfield Square, 200 YMCA Way, Penfield, NY 14526

The Gardens offer 114 affordable apartments which are owned and managed by Home Leasing. Apartments include full kitchens with dishwashers, central air-condition, in-unit storage and private balconies. The community also includes ADA-accessible units, a community room, fitness center and laundry facilities. The Gardens is adjacent to ESLC’s assisted living and memory care community, an enclosed walkway connects the buildings so that residents may benefit from programs and services provided at either community, supporting their ability to age in place. ESLC provides supportive services to 45 residents of The Gardens.

270 on East Apartments, 270 East Avenue, Rochester NY 14604

270 on East offers 112 affordable apartments which are managed by Landsman Property Management. Apartments include Energy Star Appliances, central air-conditioning and are cable and high-speed internet ready. The community also includes ADA-accessible units, a community room, fitness center, business center, storage facilities, a roof top patio and card access to the parking garage. ESLC provides supportive services to 35 residents of 270 on East.

St. Leo Senior Apartments, 171 Lake Avenue, Hilton NY 14468

St. Leo Senior Apartments are are currently under construction and will be move-in ready in January 2025. St. Leo Senior Apartments features 40 apartments, which are owned and operated through Providence Housing. Amenities include: intercom entry system and elevator access, fully-equipped kitchen with Energy Star appliances, individually controlled heat and electric, central air conditioning, in-unit storage, laundry hook-ups in unit, self-pay laundry, high-speed WiFi available in all common areas, community room with outdoor terrace; interior lounges, exercise room and free off-street parking. ESLC provides supportive services to 25 residents, all residents have access to the community center on campus for health and wellness programming, sponsored by Episcopal SeniorLife Communities.
