Pillar Medical Associates, our new medical team at ESLC, has been caring for Rochester-area seniors for more than 50 years, and is committed to helping our seniors stay healthy so that they can enjoy all that life has to offer. This commitment closely aligns with ESLC’s vision for our residents throughout our continuum of care.
Led by Dr. Brian Heppard, our new medical group has also begun an initiative to be designated as a Patient Centered Medical Home (PCMH). The Agency for Healthcare Research and Quality defines a PCMH as not just a place, but as an advanced model of primary care that delivers the core functions of primary health care through comprehensive, patient-centered, coordinated and accessible care, with new levels of quality and safety.
What this means for our residents is that they’ll receive the health care they need in the best possible way, at every level of care—with fewer transfers to emergency and hospital stays.
Pillar Medical Associates is a great partner for ESLC, because—just like us—they also believe in person-centered care and service. They understand and have expertise in the full medical, clinical and social supports that our seniors need.
However, this does not mean any of our residents have to give up relationships with their long-standing doctors. We understand the importance of maintaining these valued relationships. Our residents are free to see their own doctors, or Pillar Medical Associates at their Irondequoit or Webster office locations by appointment. If residents decide to switch to care from Pillar Medical Associates, they can rely on coordinated services from the same practice group throughout ESLC’s continuum of care. Once residents enter skilled nursing at The Episcopal Church Home, they’ll receive their care from Pillar Medical Associates.
